*You must wear a mask in the office at all times if you are not vaccinated for Covid-19.
*To limit exposure to COVID-19, ONLY PATIENTS are allowed into the office. This includes the waiting room. If the patient is under the age of 18, ONE parent/ legal guardian is permitted. If you are not a patient or a legal guardian, you will not be permitted to enter the office.
*If you have any symptoms of the virus, or if you or a member of your household have tested positive for the virus, please reschedule your appointment.
Symptoms include:
Cough
Shortness of breath or difficulty breathing
Fever
Chills
Muscle pain
Sore throat
New loss of taste or smell
*Please practice safe distancing while in the office and refrain from engaging in conversation with staff members that is not necessary for your care.
